INSURED TO DO
- Give notice of claim to Insurers as soon as possible;
- Report the incident to the Golf Club;
- If claim involving third party, Insured shall not without the consent in writing of the Insurers repudiate liability, negotiate or make any admission offer promise or payment in connection with any accident or claim.
DOCUMENTATION REQUIRED
Public Liability
- Completed Claim Form;
- Golf Club’s Incident Report or letter certifying the incident;
- Notice of Claim from third party supported by:-
a) Medical or Specialist Report as applicable } applicable to third party
b) Original Medical Bill(s) } injuries claim
c) Other incidental expenses/bills }
d) Repair/Replacement Bill(s) {applicable to third party property damage claim}
Personal Accident
- Completed Claim Form;
- Golf Club’s Incident Report or letter certifying the incident;
- Medical or Specialist Report as applicable;
- Original Medical Bills/Receipts;
Golfing Equipment & Personal Effects
- Completed Claim Form;
- Golf Club’s Incident Report or letter certifying the incident;
- Photos depicting the damaged club(s);
- Repair or Replacement Bill.
Hole-in-One
- Completed Claim Form;
- Original Hole-in-One Score Sheet;
- Certificate of Achievement from the Golf Club.
